And in the bios? Try to disable both the ACPI and APM. Turing it off only in the
kernel doesn't guarantee that there don't happen PM events in background
(especialy in notebooks).

Possible problems and solutions that come to my mind:
1. Card fond of sleeping, disable PM, use recent driver.
2. Chipset with broken PCI, turn off IDE DMA (or other busmasters),
   use a MB with a server chipset (Intel desktop chipsets usualy work too),
   move the card to another slot
3. Someting else, try to find the reason. Enable debug, run iwevent, sniff
   the traffic with another card in monitor mode and look for anything
   suspicious around the time it stops working.
